PARTICIPATION WAIVER AND RELEASE OF LIABILITY AGREEMENT In consideration of being allowed to participate in any way in the sport and activities of [Your Name/Business Name], I, the undersigned, acknowledge, appreciate, and agree that: Release of Liability: I hereby release and waive my rights to sue [Your Name/Business Name], its officers, employees, coaches, volunteers, agents, representatives, and any other individuals associated with [Your Name/Business Name] (collectively "Released Parties") from any and all liability, claims, or demands arising from injury, loss, or damage to my person or property, whether caused by the negligence of the Released Parties or otherwise. Indemnification: I agree to indemnify, hold harmless, and defend the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in the [Your Name/Business Name] activities, whether online, in-person, or otherwise. Assumption of Risk: I willingly and freely assume all risks associated with my participation in [Your Name/Business Name] activities, including but not limited to, risk of injury, illness, and damage to my person or property. PHOTO/VIDEOTAPE RELEASE I certify that my signature being affixed below on this consent form gives permission to Dream Factory the full right to use my photograph(s), videotaped image and sound byte in its marketing, public relations, promotional or instructional efforts. I willingly agreed to have my photograph(s), videotaped image, sound byte taken knowing that it could be used in various publications. I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith, or the use to which may be applied.*

RETURN/REFUND/MAKE-UP POLICIES There are absolutely no refunds unless there are extenuating circumstances (i.e. Family Emergencies, Health Concerns, etc.). We expect you to complete your sessions within each month and we look forward to motivating you to do so. Sessions do NOT rollover. Sessions must be cancelled 24 hrs prior failure to do so will result in that session counting towards your package. *
